Compassion, experience and personalized support make it possible for older adults to remain safely and comfortably in their own homes.

Within Chautauqua County Office for Aging Services (CCOFAS) organization, seven dedicated Case Managers bring a combined 80 years of experience working with the elderly population. Their knowledge, compassion and commitment play an essential role in helping older adults maintain their independence, dignity and quality of life while remaining in the comfort of their own homes.

As individuals age, everyday tasks and responsibilities can become increasingly challenging. Access to services and support can make a significant difference in allowing older adults to continue living independently rather than transitioning to a nursing home or other residential care setting.  Case Managers work closely with each client to identify needs, develop personalized care plans and coordinate services that support safe and successful aging in place. These services may include aides to assist with housekeeping and/or personal care, social adult day care, personal emergency response systems (PERS), home delivered meals and other resources that promote independence.

The combined 80 years of experience among our Case Managers represent a valuable depth of knowledge and understanding of the unique challenges faced by older adults. Their experience allows them to recognize changing needs, advocate for services, address potential barriers and help them and their families navigate available resources.

Beyond coordinating services, our Case Managers build relationships with the clients they serve. They have regular contact with their clients and understand that home is more than a place to live; it represents familiarity, comfort, independence and treasured memories. By listening to each client’s concerns and respecting their choices, they help older adults maintain a sense of control over their lives while providing reassurance to families and caregivers.

Our organization’s commitment to supporting older adults in their homes reflect the belief that aging should not mean giving up independence. With the guidance of experienced Case Managers, they can access appropriate community-based services and many older adults can continue to live in their homes safely while receiving the assistance they need.

The seven Case Managers within our organization are an important part of making this possible. Their experience, dedication and compassion demonstrate our ongoing commitment to helping older adults age in place for as long as safely possible. Through their work, they make a meaningful difference in the lives of individuals, families and communities we serve.

Together, our Case Managers help turn the goal of aging in place into a reality-one individual, one care plan and one home at a time.
