NY Connects

 

WHAT is NY CONNECTS?

NY Connects is a No Wrong Door (NWD) system 

that enables older adults and their families to identify available services and supports; 

and choose options that will best meet their needs at home.

 

In short, NY Connects is the point-of-entry program 
to Office for Aging Services
 to help you assess your or your family member's needs , 
identify the most appropriate services to meet those needs, 
and help you link to the agency or program that will meet those needs.  

 

NY Connects consists of the following: 

  • Information and Assistance:

    Provides Information and Assistance on available long-term services and supports through phone calls to 716-753-4582, face-to-face meetings with a NY Connects Operator at one of our three offices, or email to CCNYC@chqgov.com.  Additionally, there is an online resource directory of providers that provides information about the different types of services and contact information for local programs.   The directory is free to utilize and helps contact you to  the agencies and information when looking for services throughout Chautauqua County.  

  • Person-Centered Options Counseling: 

    This is an interactive, goal driven person-centered process whereby individuals are supported in exploring options in making informed long-term support decisions based on their preferences, strengths, values, abilities, and resources. 

  • Public Education:

    Check out the NY Connects table or tent at local events or fairs.  We make it easy for you to get connected to the resources you need. 

  • Long-Term Care Council:

    Long-Term Care Councils are made up of older adults like you, caregivers, service providers, advocates, government representatives, and key stakeholders who come together to analyze the local long-term care services and supports in our local community.  They identify gaps, duplication, and barriers in the system and make recommendations for improvements.   The findings help individuals remain at home and in the community.
