MAYVILLE, N.Y. — The Chautauqua County Health Department (CCHD), in collaboration with Brooks-TLC Hospital System, Inc., UPMC Chautauqua, and Allegheny Health Network Westfield Memorial Hospital, is pleased to announce the completion of the 2025–2030 Community Health Assessment (CHA) and Community Health Improvement Plan (CHIP).
CCHD extends its sincere appreciation to all members of the Chautauqua County Community Health Planning Team (CCCHPT) for their collaboration, commitment, flexibility, and willingness to explore innovative approaches to improving community health. The CCCHPT includes representatives from UPMC Chautauqua Hospital, Brooks-TLC Hospital System, Inc., Allegheny Health Network Westfield Memorial Hospital, The Chautauqua Center, The Resource Center, Chautauqua County Office for Aging Services, The Heritage Group, Prevention Works, the Chautauqua County Department of Mental Hygiene and Social Services, Chautauqua Health Network, and the Healthy Community Alliance.
The 2025–2030 CHA and CHIP align with the New York State Prevention Agenda, the state’s health improvement plan that serves as a comprehensive framework for local and statewide action to improve health outcomes and reduce health disparities. Developed and updated by the New York State Public Health and Health Planning Council, the Prevention Agenda emphasizes prevention and equity, with the vision that every New Yorker has the opportunity to achieve their highest level of health across the lifespan.
This marks the fourth cycle of the Prevention Agenda since its inception in 2008. New for the 2025–2030 cycle, the framework was revised to reflect the growing impact of socioeconomic factors on health and now aligns with the five social determinants of health outlined in Healthy People 2030.
Using data from the community health assessment, the CCCHPT is committed to educating residents and stakeholders about key health trends and challenges affecting Chautauqua County. The assessment focuses on the following five priority areas:
- Economic Stability
- Social and Community Context
- Neighborhood and Built Environment
- Health Care Access and Quality
- Education Access and Quality
